Objective and notes from the draft
To assess whether coronary revascularization (PCI or CABG) plus medical therapy, compared with medical therapy alone, reduces death, cardiovascular events or kidney harms in people with non-dialysis-dependent chronic kidney disease and ischemic heart disease, as shown in randomized trials and their subgroup or post hoc analyses.
- The restriction to non-dialysis-dependent CKD cannot be searched reliably, because subgroup analyses rarely state dialysis status in indexing; dialysis patients must be excluded at screening.
- Randomized design is searched sensitively (randomization, trial, post hoc, trial headings) rather than with the Randomized Controlled Trial publication type, so that CKD subgroup and post hoc analyses indexed as cohort or retrospective studies, such as PMID 22728005, are not lost.
- The outcome block is optional and not combined: the question's outcomes (death, cardiovascular events, kidney harms) are reported in nearly all relevant trials and adding them would lower sensitivity.
- Optimal medical therapy is not searched, as the comparator is implied by trials of revascularization versus medical therapy.
- CAD is not used as an abbreviation because it also means computer-aided design or detection.
- The renal impairment block includes eGFR and renal function phrases so that trials reporting results by kidney function strata are found; this adds some noise that screening must remove.
